In line with public health directives regarding the ongoing Ebola outbreak, all passengers arriving from or transiting through the countries listed below must complete the Traveler Health Surveillance Form prior to travel:

Passengers must register, complete the form, and obtain a QR code. This QR code must be presented to Port Health officials upon arrival.

Uganda Travel Advisory – Health Declaration Requirement
Following the recent Ebola alert issued by the Ministry of Health Uganda, all travelers arriving into or departing from Uganda are required to complete the official Passenger Health Declaration Form prior to travel.
This requirement applies to all points of entry and exit, including airports and border crossings.
