Continuum Care staff is committed to ensuring the end-of-life journey is comforting and peaceful for both the patient and their loved ones. Our team of nurses, social workers, chaplains, aides, Certified Music Therapists, Certified Massage Therapists, volunteers, and Medical Directors is to enhance the quality of life for patients with life-limiting illnesses and provide their loved ones with the support they need for the duration of the illness.
