Enrollment Questionnaire 1

You’ll begin your checklist with the Enrollment Questionnaire. This form collects basic information, including your current address, phone number, personal email address, preferred pronouns, and even your T-shirt size and dietary restrictions. Our goal is that once this form is completed, you won’t need to enter your contact information on any additional forms. If you have changes to report between the time you complete this questionnaire and your move to campus in August, contact the Ofce of Admission at admission@denison.edu.

Family and guardian information is used by a variety of campus ofces, including Student Life, Institutional Advancement, and First-Year Experience. Complete this form to make sure your loved ones stay up to date on all things Denison.
